Semen analysis and blood tests to evaluate male factor: The
semen analysis specifically determines the sperm concentration, motility (movement)
and morphology (shape).
Tests to evaluate the uterus and tubes. Hysterosalpingogram (HSG) can
uncover the presence of uterine scar tissue, polyps, fibroids, or an abnormally
shaped uterus. The X-ray offers detailed images of the uterine cavity and the
fallopian tubes. A hysteroscopy, which uses a fiberoptic scope,
may also be performed to conduct an examination of the uterus.
Laparoscopy can diagnose tubal factor; such as blocked or
damaged fallopian tubes. The procedure enables a surgeon to view and find abnormalities
of the uterus, ovaries, fallopian tubes, and pelvis using a fiberoptic scope.
Damaged or blocked fallopian tubes can sometimes be repaired via endoscopic
surgery, using laser; electrocautery; or other operative techniques.
Pelvic ultrasound can determine if a woman has irregular
or abnormal pelvic anatomy, as well as provide information about the uterus
and its endometrial lining. Using advanced ultrasound technology, a doctor can
see if the ovaries are producing follicles, the cystic structures where the
eggs develop, or if the uterine lining is adequate. Saline infusion sonohysterography
(SIS) can delineate intrauterine problems.
Postcoital test (PCT) evaluates cervical function by analyzing
mucus, sperm, and the interaction between the two.
In Vitro Fertilization (IVF)
Eggs are retrieved from the ovary and fertilized with sperm from a male partner
or donor. Fertilization occurs in the laboratory outside the woman's body. The
embryos are then transferred to the uterus. This is a promising option for couples
whose infertility is caused by blocked, damaged, or absent fallopian tubes;
male factor; endometriosis; or unexplained infertility.
Gamete Intrafallopian Transfer (GIFT)
GIFT is an option for women who have healthy fallopian tubes.
In this procedure, the retrieved eggs are placed with sperm in the fallopian
tubes of the woman using laparoscopy. Fertilization occurs in the fallopian
tubes rather than in the laboratory. The embryo travels through the tubes into
the uterus where implantation and pregnancy can occur.
Intracytoplasmic Sperm Injections (ICSI)
Severe male factor infertility can be overcome with ICSI, a highly successful
micro-injection technique that introduces a single sperm into the egg.
