ERAS Fellowships: Transcript and MSPE (Dean’s Letter) Requests
Graduates of New Jersey Medical School seeking a transcript and MSPE for the purpose of application to fellowship positions must submit the corresponding NJMS Transcript and MSPE Request Forms (see below) and remit payment for the documents. Once we receive the forms and payment, we will upload your documents directly to ERAS using the ERAS Medical Institution Document Upload Service.
When applying to fellowship programs through ERAS, you will be prompted to identify a staff member to whom the upload link should be sent. Please have these requests sent to Carrie DeVincens, firstname.lastname@example.org. Requests for official academic transcripts can be made online, via mail or in person. All former/previously enrolled students will be charged a $7.00 transcript fee for each Official Transcript ordered. All students enrolled at the time of the transcript request will receive two free transcripts; thereafter each additional request will generate a charge of $7.00 per transcript.

If you wish to make your request via mail or in person, please download and complete the Transcript Request Form below. Completed forms must be mailed or delivered to the Registrar's Office; requests must include the payment noted above. Transcripts will be processed within seven business days after the request and payment are received. Fees are payable by check or money order addressed to "Rutgers University."
If you have supplemental forms to submit for inclusion with the academic transcript request, please send them to this office for processing.
MSPE (Dean's Letters)
Requests for official dean's letter can be made via mail or in person by downloading and completing the Dean's Letter (MSPE) Request Form below. Completed forms must be mailed or delivered to the Registrar’s Office with a $5.00 fee per dean's letter. Letters will be processed within five business days after the request and payment are received. Fees are payable by check or money order payable to NJMS.
Transcripts, dean's letters, and diplomas are not issued for students with outstanding debts to the University. The Registrar’s Office will not fax transcripts or dean's letters to the requestor or a third party.
