Requests for Reports from University Hospital Medical Records

Reports from the medical record system must be requested via the UH Universal Report Request Portal, found at: https://research.njms.rutgers.edu/redcap/surveys/?s=CCCFKNKTD7

Note that if the report is related to research (OR if it is for a Quality Improvement project which meets the criteria for a non-human subject determination), University Hospital requires a fully executed UH research plan prior to generating the report. Remote Monitoring Visits- Access to Epic for Monitors

Access to Epic via EpicCare Link may be granted to study monitors for remote monitoring visits upon request. Study teams may request assistance with arranging access for their monitors by contacting Arleen Wallen wallenar@uhnj.org or Eduardo Torres torresed@uhnj.org
