University Hospital Chart Review Registration

If you are conducting research that requires use of University Hospital medical records you will need to register the study with UH. The first step is to e-mail OCRAreview@njms.rutgers.edu the draft version of your protocol which you plan to submit to the IRB, as well as completed and signed UH Chart Review Registration Form and the UH Research Plan. Once received, OCRA will reply with a receipt which may be uploaded to your eIRB application as proof of notification to NJMS-OCRA and University Hospital.  The receipt will also contain a checklist for the process.
