Slide 41:  Acute Proliferative Glomerulonephritis, with Early Crescents

PERTINENT CHANGES

  1. The capsular surface is not depressed.
  2. Glumeruli:
    1. There is an increase in cellularity.
    2. Capillary lumens are obliterated.
    3. The glumerular tufts are segmented.
    4. Many glomeruli show an increase of epithelial cells lining Bowman's capsule (crescents).  Some form a narrow rim others are several layers
  3. Tubules:
    1. Proximal convoluted tubules show vacuolar changes.
    2. Lumens of tubules contain smudgy pink material, neutrophils, and desquamated epithelial cells.
  4. Stroma is light pink and edematous.

A.  The emphasis in this slide is on the proliferative glomerular changes as shown on this high power view of a glomerulus.  There are increased numbers of cells in the glomerulus partially obliterating its capillaries.
