Error Reporting for all 3 sites

Since 1999 the Institute of Medicine has recommended that patient safety reporting systems be used in an effort to improve patient safety.  Since 2004 the State of NJ has required that all acute care hospitals report medical errors to the New Jersey Department of Health for use in developing patient safety initiatives.  

All three sites have mechanisms in place for reporting patient safety concerns.  Each event reporting system has an extensive list of reportable events however common occurrences include medication dosing or administration errors, delays in care, complications of lab testing, and near miss events.
