Error Reporting for all 3 sites

Since 1999 the Institute of Medicine has recommended that patient safety reporting systems be used in an effort to improve patient safety.  Since 2004 the State of New Jersey has required that all acute care hospitals report medical errors to the New Jersey Department of Health for use in developing patient safety initiatives.  

All four sites have mechanisms in place for reporting patient safety concerns.  Each event reporting system has an extensive list of reportable events however common occurrences include medication dosing or administration errors, delays in care, complications of lab testing, and near miss events.

Cooperman Barnabas Medical Center

  • Click the "Verge" icon located on every computer desk top to anonymously report adverse events, near misses, and errors; also available on the Bridge.
  • Call the Patient Safety Hotline at 973-322-2082.

 

University Hospital – Riskonnect

  • Log in to Riskonnect via Citrix Workspace or EPIC (click on “Clinical Links” on top and look for the Riskonnect icon on the webpage that opens)

 

Veterans Affairs Medical Center

  • Log-in to any VA computer and choose the "Electronic Patient Event Report" program located on the desktop.
