Thesis Mentor Approval Form

A successful student-mentor relationship requires commitment from the student, mentor, graduate program, and institution. The Thesis Mentor Approval Form is the first step in formalizing the agreement between the student and mentor. The AAMC Compact between Biomedical Graduate Students and their Research Advisors offers a set of broad guidelines which are meant to initiate productive discussions to ensure the student’s progress through the degree. The Graduate School expects all students and mentors to abide by these guidelines. By signing the Ph.D. Thesis Mentor Approval DocuSign form, the student and mentor confirm that they have read the compact and have met to discuss the salient features; specifically including the 'Commitments of Graduate Students' (pages 5-6) and the 'Commitments of Research Advisors' (pages 7-8).
