Important Forms
Note: These forms are for current MD PhD Students, particularly those in their PhD training years.
Candidacy Exam
Candidacy Examination Committee Approval - This form is to be initiated by the Track Director.
Candidacy Examination Report - This form must be submitted to the Graduate School within one week following the examination. This form is to be initiated by the Candidacy Exam Committee Chair.
Thesis Advisory Committee, Thesis Examination Committee, Thesis Defense
Thesis Advisory Committee Nomination Form - This form is to be initiated by the student
Thesis Advisory Committee Meeting Form - This form is to be initiated by the student
Dissertation Examination Committee Form - This form is to be initiated by the student
Report of Final Examination/Dissertation Defense Form - After the public oral defense, each member of the final examination committee signs a Report of Final Examination/Dissertation Defense Form attesting to the outcome. This form is to be initiated by the student.
Registration
Registration form (Use this form if you have not registered for courses yet.) This form is initiated by the student
Add Course Form (Use this form if you currently registered for courses and would like to add additional courses.) This form is initiated by the student
Withdraw/Drop Form This form is initiated by the student.
Cross-Registration with New Jersey Institute of Technology (NJIT)
Obtain a CHEN Registration Form and permission from your departmental Graduate Program Director to take the course. This form is to be initiated by the student
Student Travel Awards during the PhD Phase
Requests for travel award must be accompanied by a Rutgers Travel Approval Report.
Pre-Candidacy Student Travel
- Rutgers Travel Approval Report: Student initiates this form through the graduate school.
- They must be in good academic standing.
- They must be attending a scientific meeting in the 2nd or 3rd semester. There are some exceptions to this requirement.
- Travel approval eligibility is with the permission of the track director and SGS budget office.
- Pre-Candidacy Student Travel Award Approval Form: Student initiates this form through the graduate school with the completed Travel Approval Report attached.
- They must be in good academic standing.
- They must be traveling with a faculty/post-doc to help them maneuver a scientific meeting.
- Must have departmental budget approval of the faculty to cover any eligible expenses that exceeds the travel award.
- Travel is arranged through the graduate school.
Post-Candidacy Student Travel
- Rutgers Travel Approval Report: Student initiates this through their PIs department. They must follow the travel approval policies of the department.
- Post-Candidacy Student Travel Award Approval Form: Student initiates this form through the graduate school with the completed Travel Approval Report attached and formal abstract attached.
- They must have an approved thesis proposal and presenting a paper/poster at a scientific meeting.
- They must have a formal abstract acknowledging Rutgers School of Graduate Studies as the research site.
- Research must be part of the approved dissertation proposal.
- Travel is arranged through the PIs department and the graduate school will transfer the award funds upon completion of travel.
