4 year combined Internal Medicine - Nephrology residency
The Combined Internal Medicine-Nephrology Track is our newest addition to the program, with its first enrollee in July 2025. As one of only 3 programs in the country approved by the ACGME to offer this under a pilot study, this novel track combines Internal Medicine Residency and Nephrology Fellowship (normally at least 5 years of training) into an accelerated 4-year timeframe. Residents in this track will receive enhanced Nephrology rotations, curricula, and outpatient clinic experience from the beginning of training, in order to facilitate dual board-certification by the end of the 4th year. For those interested in a Nephrology career, this track is a great opportunity to personalize your post-graduate training experience, reduce your training time, and guarantee fellowship placement without the need to apply through the fellowship ERAS cycle.
Curriculum
Summary of Educational Experiences by Training Year:
- Track participants rotate in the same 4+2+2 schedule as the other categorical residents.
- From the beginning of residency, track residents have 25% of their continuity ambulatory experience scheduled in the nephrology clinic at University Hospital.
- The PGY-1 resident has a very similar schedule to a traditional categorical intern. They have 2 elective rotations assigned to nephrology, at minimum, where they function as resident consultants and learn the basics of the field.
- PGY-2 residents similarly have a traditional categorical schedule, with a minimum of 3 elective nephrology experiences across our 3 training sites- inclusive of rotations in hepatorenal disease and graduated responsibility in our consult services.
- PGY-3 trainees have a hybrid year that features a transition to fellowship halfway through the year. As a resident, the trainee has experiences in renal transplant and outpatient nephrology. As a fellow in the second half of the year, the trainee will assume fellow-level responsibilities on consult rotations.
- PGY-4 is a year devoted to completing the requirements of a traditional nephrology fellowship.
- The trainee is board eligible to take the ABIM Internal Medicine Certification Exam with the rest of their class in the summer of their 4th year. After completing the 4th year, the trainee is eligible to then sit for the Nephrology Boards.

How to apply
- We plan to enroll 1 resident per year in the combined IM-Nephrology track.
- The track has a separate heading in ERAS. Please apply to Rutgers Health- New Jersey Medical School and select “Internal Medicine-Nephrology 4-Year Track.”
